About this book

Looking after someone means keeping track of everything: which medications were taken and when, what was eaten, how the day went, what the doctor said, what changed since last week. This log book keeps all of it in one place, on paper, where everyone on the care team can read it.

Each daily care sheet uses the same fixed fields: blank date and caregiver lines, a medication log with medication, dose, time given, and taken checkboxes, meals and hydration, vitals and observations, activities, appointments, incident notes, and a notes-for-tomorrow box. Four weekly summary pages ask the useful questions: what went well, what was concerning, and what to report to the care team.

The book opens with a care recipient profile and emergency contacts page, then a medication schedule template and a doctor-visit worksheet with questions to ask and space for the doctor's answers. A supply checklist and an appointment log round it out.

Every page is undated. Start any day, print extra sheets when weeks run long, and hand the book to the next caregiver with the whole picture intact.
